This site provides a brief summary of the employee benefit plans in effect on January 1, 2018 for the Practice Groups in California that are included in the MED3OOO Group, Inc. Multiple Employer Welfare Arrangement (MEWA). It is not a Summary Plan Description (SPD). However, this site serves as the "Summary of Material Modification" to our benefit plans in accordance with the requirements of the Employee Retirement Income Security Act of 1974, as amended (ERISA). If there is a discrepancy between this site and the applicable insurance contract, agreement, SPD or plan document, the applicable insurance contract, agreement, SPD, or plan document will prevail.
